Dermi 5 is a combination of 0.05%w/w of Topical Clobetasol propionate, 0.1%w/w of Topical Gentamicin, 1%w/w of Topical Clotrimazole, 1%w/w of Topical Clioquinol (Iodochlorhydroxyquin) and 1%w/w of Topical Tolnaftate. The ointment is rapidly absorbed from the site of application and acts quickly to bring down inflammation and swelling. The ointment is minimally absorbed and hence is generally tolerated well.
The Dermi 5 belongs to the category of corticosteroids and works as an anti-inflammatory agent. It blocks the metabolism of arachidonic acid by obstructing synthesis of phospholipase A2. This results in the reduction in the production of inflammatory agents such as leukotrienes and prostaglandins.
Clobetasol propionate is a highly potent topical synthetic glucocorticoid with excellent anti-pruritic and anti-inflammatory properties. It works by suppressing the action of inflammatory agents that cause built-up of fluid resulting in swelling, induration and redness. It suppresses tissue factors that cause increased itching in affected areas. It has vast-constrictive properties too that help to reduce redness, warmth and inflammation of afflicted areas.
Gentamicin is a topically effective antibiotic which works by interrupting protein synthesis in the causative bacteria. Thus, there is arrest in multiplication of bacterial organisms resulting in good antiseptic action.
Clotrimazole is an azole anti-fungal which acts by interfering with the biosynthesis of ergosterol which is an essential component of fungal cell membranes. This results in stopping of fungal proliferation in affected sites.
Clioquinol is a potent anti-bacterial, anti-fungal and anti-protozoal agent which acts as a topical antiseptic. The exact mechanism by which Clioquinol exerts its action is unknown.
The Dermi 5 is prescribed to treat following conditions:
Dermi 5 Cream has no reported interactions with any other medication. The steroid component may increase the severity of Diabetes mellitus in patients diagnosed with the condition. Dose adjustment is required as the effects are directly proportional to the concentration of the steroid in the ointment applied. Consultation with a registered medical consultant is essential if Dermi 5 application is necessary in patients suffering from or is at high risk of developing Diabetes mellitus.
Research studies on animals have revealed adverse fetal effects on topical application of Dermi 5. Hence Dermi 5 is not recommended for use in Pregnant or lactating mothers unless the benefits substantially outweigh the risks. In any case, the ointment should be used only after consultation with a registered medical practitioner. Studies have shown that there is no significant interaction with alcohol intake.
